Dear Parents:

The Jewish Community Day School will once again be sponsoring a milk program for our students, beginning on Tuesday, September 11th. You now have the opportunity to pay a one-time charge of $100.00 to provide your child with milk at lunch, five days a week, for the entire school year. Please note that there can be no credit issued for milks missed due to the absence of students, cancellation of school or on Hot Lunch Day.

We will be offering low-fat milk supplied by Guida’s Dairy and the milk will be delivered fresh every Tuesday.

If you are interested in having your child participate in this program, you MUST SIGN UP BY FRIDAY, SEPTEMBER 7TH. Please complete the form below and return it to the school office, along with your check made payable to the Jewish Community Day School. Please be sure the milk payment is a separate check and not combined with any other payment.
